Tirzepatide and Hypothyroidism: What You Should Know

Tirzepatide, sold under the brand name Mounjaro, is one of the most promising new treatments for type 2 diabetes and weight management.

With many patients who have hypothyroidism also struggling with weight gain, it’s natural to ask: can tirzepatide help, and does it affect thyroid health?

Recent studies show that tirzepatide leads to significant weight loss, often 15–20% of body weight over 72 weeks and this amount of weight reduction can influence thyroid function, especially in people taking thyroid hormone replacement.

That’s why patients with hypothyroidism are paying close attention.

SEE HOW TIRZEPATIDE CAN HELP YOU

Can a Person With Hypothyroidism Take Tirzepatide?

Yes, most people with hypothyroidism can safely take tirzepatide.

The medication itself does not harm the thyroid or interfere with thyroid medications such as levothyroxine.

In fact, by supporting weight loss, tirzepatide may indirectly reduce the metabolic strain caused by an underactive thyroid.

What’s important to know is that significant weight loss can change your thyroid hormone requirements.

Some patients find that their levothyroxine dose needs adjustment as they lose weight.

That’s why regular thyroid bloodwork is essential after starting tirzepatide.

Does Mounjaro Affect Thyroid Function Directly?

The evidence so far suggests that Mounjaro does not directly change thyroid hormone production in most people.

Instead, any thyroid-related changes are usually secondary to weight loss.

  • In one case report, a patient experienced a thyroid crisis after very rapid weight loss with tirzepatide, but this is extremely rare.
  • Clinical trials did not show a consistent pattern of thyroid dysfunction among thousands of participants.
  • For the majority of patients, thyroid function remains stable, though medication adjustments may be needed.

Can You Take Tirzepatide if You Have Hashimoto’s?

Yes, patients with Hashimoto’s hypothyroidism can generally use tirzepatide.

Because Hashimoto’s is an autoimmune condition, tirzepatide will not stop the immune attack on the thyroid itself.

However, it can improve weight control, insulin sensitivity, and cardiovascular health, which all support better long-term outcomes for people with Hashimoto’s.

Doctors recommend more frequent thyroid hormone checks for Hashimoto’s patients on tirzepatide to ensure medication dosing stays in the right range.

Does Tirzepatide Increase the Risk of Thyroid Cancer?

One concern that comes up often is whether tirzepatide raises thyroid cancer risk.

Here’s what we know:

  • Animal studies found that GLP-1 medications like tirzepatide were linked to certain thyroid tumors in rodents.
  • Human studies have not confirmed this risk. Large clinical trials of tirzepatide have not shown an increased rate of thyroid cancer.
  • Exceptions exist: People with a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN2) are typically advised not to take GLP-1 medications.

For most patients with hypothyroidism or Hashimoto’s, the cancer risk is considered very low.

How Does Tirzepatide Compare to Semaglutide for Thyroid Patients?

Both tirzepatide (Mounjaro) and semaglutide (Ozempic/Wegovy) are GLP-1–based medications that support weight loss and blood sugar control.

Some studies suggest tirzepatide leads to slightly greater weight loss, which may also mean more noticeable effects on thyroid medication needs.

Neither medication has been shown to worsen hypothyroidism directly.

The choice between the two usually depends on availability, cost, and individual health goals rather than thyroid-specific concerns.

What Should You Do If You Have Hypothyroidism and Want to Start Tirzepatide?

If you’re considering tirzepatide and have hypothyroidism, here are the steps to take:

  • Get baseline labs to check TSH, T3, and T4 before starting.
  • Recheck thyroid labs after 8–12 weeks on tirzepatide, and again after major weight changes.
  • Watch for symptoms like unusual fatigue, heart palpitations, or mood changes, which may signal your thyroid medication needs adjustment.
  • Work closely with your doctor to fine-tune both treatments together.

Conclusion: Tirzepatide and Hypothyroidism

Tirzepatide and hypothyroidism can safely coexist for most patients.

The medication doesn’t directly interfere with thyroid function, but significant weight loss may change thyroid hormone requirements, making close monitoring important.

For patients with Hashimoto’s or other thyroid conditions, tirzepatide may support overall health by improving weight, blood sugar, and cardiovascular markers.

At Eternity Health Partners, we help patients explore treatments like tirzepatide while carefully managing conditions like hypothyroidism to ensure safe and lasting results.

FAQs: Tirzepatide and Hypothyroidism

Does tirzepatide affect your thyroid?
Tirzepatide does not directly affect thyroid hormone production in most people. Its main impact comes from weight loss, which can change how your body responds to thyroid medication. Regular monitoring helps ensure thyroid levels stay balanced.

Can you take weight loss injections with hypothyroidism?
Yes, many people with hypothyroidism can safely use weight loss injections like tirzepatide or semaglutide. The key is to monitor thyroid hormone levels closely, as significant weight loss may require medication adjustments.

Can I take tirzepatide with no thyroid?
If your thyroid has been removed and you are on thyroid hormone replacement, you can still take tirzepatide. Your doctor may need to adjust your thyroid hormone dose if your weight changes significantly.

Can I take GLP-1 if I have hypothyroidism?
Yes, GLP-1 medications such as tirzepatide and semaglutide are generally safe for people with hypothyroidism. Your provider will monitor your thyroid function and adjust medications if needed.

Can you take semaglutide with hypothyroidism?
Semaglutide can also be used by patients with hypothyroidism. Like tirzepatide, it may help with weight management, but close monitoring of thyroid hormone levels is recommended.

What is the fastest way to lose weight with an underactive thyroid?
The fastest and safest way to lose weight with hypothyroidism is a combination of well-managed thyroid medication, a balanced diet, regular exercise, and in some cases, medical therapies like GLP-1 injections. Crash diets or unmonitored treatments can disrupt thyroid balance and are not recommended.

What cannot be taken with tirzepatide?
Tirzepatide should not be combined with other medications that may cause dangerously low blood sugar unless managed by a doctor. It is also not recommended for people with a history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2. Always review all prescriptions and supplements with your provider before starting tirzepatide.

What are the benefits of tirzepatide vs semaglutide?
Both tirzepatide and semaglutide support weight loss and blood sugar control. Tirzepatide may lead to greater weight loss in clinical studies, while semaglutide has longer-term safety data. The right choice depends on your health profile and provider recommendations.

MEET ELIZABETH BECKNER AND DAISHA ESCAMILLA

OUR CALIFORNIA LICENSED PHLEBOTOMISTS

Liz started doing personal blood draws after her son, Dashiell, was born with Zellwegers disease. His diagnosis required numerous blood draws and drug levels throughout his life. After sitting through long wait times at the labs followed by watching many different phlebotomists, many new to the job, struggle to find his veins, often poking him multiple times, as a seasoned phlebotomist, Liz decided to draw his blood at home and hand deliver it to the lab myself. Soon after she began doing personal touch blood draws so she can now offer that same personalized service to you.

Liz graduated from Pepperdine University with a degree in Biology in 1994. She moved to Santa Barbara and earned her phlebotomy certificate in 1995. Liz began working at St. Francis Medical Center in the Physical Therapy Department and quickly transferred to the Laboratory. Perfecting her venipuncture techniques on all floors of the hospital, including post-operative patients, pregnant women going into labor, sick or premature babies, and emergency room patrons, she then improved her patient care skills working for internal medicine doctors, one of whom was the Medical Director of Hospice and won Doctor of the Year. In 1999, Liz started working with a group of local retinal surgeons. Her primary job consisted of injecting dye into arm veins and photographing its flow through the patients retinal vessels. When patients are worried about their diagnosis or apprehensive about the procedure because of difficult veins, her quick, almost pain free needle stick and kind but professional bed side manner would put them at ease.

The various settings and patients Liz has encountered over the years have enabled her to become very skilled at finding the smallest and most fragile of vessels. Her specialities include infants and the elderly, diabetics of all ages, patients with rolling veins, on blood thinners or chemotherapy. She lives in downtown Santa Barbara, travels from Goleta to Carpintera and brings all the necessary supplies directly to you. All she needs from you is a lab request and your home or office becomes an instant draw station. Appointment times are very flexible, and she is friendly, reliable and punctual. MEET MICHAEL BROOKINS; BS, CNMT, ABAAHP

Mike brings more than just a medical perspective to his clients; at the age of 14, he began an athletic career as a pole-vaulter that would end up taking him on an incredible journey over the next 13 years of his life. Throughout his athletic journey Mike was constantly working to find the ideal balance of fitness and proper nutrition in order to maximize his performances. Combined with his medical knowledge the years of athletic experience provide a unique and successful combination that serve to benefit his staff and clients.

“After I shifted the focus of my practice from Sports Medicine into Nuclear Medicine specifically Interventional Endocrinology. I found myself questioning our western medical paradigm…Here I was performing thyroid cancer and Hyperthyroidism treatments with harmful radical thyroid radioactive ablations which really made me take a hard look in the mirror and become more of a researcher to find safer and more effective options to better treat my clients. During my research I realized that there were very few, if any, medically directed programs focused towards achieving a “Healthier Lifestyle” rather than typical “Diet” fads.” A few years later after training and working with some of the worlds foremost Bio-identical Hormone Replacement specialists I became Board Certified in Anti-Aging and Regenerative Medicine. Sort of just found my passion and the answers I had been looking for with this new specialty finally after years of treating people with the Band-Aid “fix it when its broken” mentality instead of at the root cause.”

Mike has spent the last 10 years in Anti-Aging, Functional and Regenerative Medicine. He has trained over 120 physicians in Anti-Aging and Regenerative Medicine and consulting for their practices. Today his vision is a reality, and clients from all of the world come to benefit from the successful programs that have been established at Age Management Institute Santa Barbara. He firmly believes in educating the patient about the importance of good nutrition, hormone balance, stress management, and regular exercise is the key to aging well and having a high quality of life well into your golden years.
