MEMBERSHIP AGREEMENT GUIDELINES

We are excited to be working with you on your wellness goals and tailoring your program to fit your needs. As a unique integrative health and wellness organization with our preferred Physician at Eternity Health Partners will be partnering with you to help you make positive changes in your life that you feel will help you live life to the fullest.

MEMBERSHIP AGREEMENT

You have chosen to enter into a membership program with Eternity Health Partners. This is a program tailored to your specific goals and is important for you to keep in contact with the Eternity Health Partners Team to ensure you the best care for your treatment plan. Your monthly membership includes the following:

  • Approved Medication Costs Prescribed by Preferred Physician (see your Membership Agreement Form which medications are included in your membership)
  • Medication Shipped directly from the pharmacy to your door

  • Concierge Medication refill service (we communicate to the pharmacy on your behalf)

PRESCRIPTIONS

All medications will be shipped directly to you from the pharmacy via ground shipping (7-10 business days) which is included in your membership. When you receive the medications be sure to read the instructions provided from the pharmacist and dosing prescription. The staff at Eternity Health Partners are also available to schedule a consultation to review dosing and administration. It is the patients responsibility to request refills for their prescriptions. Prescription refills are NOT automtic.

* It is important that you follow the Doctor prescribed dosing protocol that is written on the medications from the pharmacy.  The pharmacy will not refill the prescription early if you are taking more of a dose than the doctor has prescribed. 

*If medication is lost, confiscated and/or damaged, you are responsible for the pharmacies restocking cost and for the price of the replacement of medication.

PRESCRIPTION REFILL REQUESTS

It is the patient’s responsibility to request refills for their prescriptions. For prescription refills, we ask that you contact our office 7-10 days before running out of your medication, for your medication refill request. Please plan ahead to avoid any interruptions in your medications.

* It is important to be aware that prescriptions refills are NOT automatic.

PAYMENT OPTIONS

SHIPPING FEE

If you prefer to expedite your Prescription from the pharmacy you can pay the additional $30 for 2 Day Priority Shipping, or $50 for Overnight Service. *Please note that all prescriptions require a 2 day pharmacy processing time. If you need a rush on your prescription there is a $10 charge.

SERVICE FEE

There is no sales tax in the state of California for medications, however you may be subject to a 4% pharmacy/merchant fee.

RETURNS / REFUNDS

Failure to follow the Protocols for your membership program does not relieve you of the obligation to pay the full tuition for the program or phase in which you are currently enrolled. Over the counter supplements purchased with Eternity Health Partners powered by Age Management Institute may be returned for a refund or exchange if in the original condition and unopened or unused within 14 days of purchase. No prorated refunds are provided for cancellations made before the end of the paid-up period.  For all refund requests there will be a total of $25 transaction fee that will be deducted from the refund. Refund requests must be submitted in writing to our Client Service Coordinator and may take up to 10 days to process. Refunds will be submitted via check to your provided mailing address

PRIVACY

Eternity Health Partners is committed to protecting the privacy of our clients. We make every effort to ensure that the information you share with us is recorded accurately, retained securely and used only according to your wishes. If you have any questions about how your personal information is handled or distributed, please contact our office.

FREEZE AND/OR CANCELLATION

We understand that circumstances may occur during your membership and you have 2 options in what to do with your membership. To cancel or to freeze the account both of which require a written notice, delivered to Eternity Health Partners by mail, email or in person at least 15 days prior to the cancellation/freeze date desired.

Cancellation: Your membership agreement with Eternity Health Partners may be canceled at any time. Once the 15 day written cancellation notice has been processed, billing will be stopped and your chart will be closed. In order to get back into any program with Eternity Health Partners, you will have to rejoin as a new patient with a face to face consult with our Physician, including and not limited to a physical exam and new labs. 

*NOTE: The cancellation date will depend on the most current refill date of your prescription. Any or all Membership Discounts will be discontinued upon cancellation request. 

Freeze: A membership with Eternity Health Partners may be placed on hold at any time of up to 90 days for $30/month, limited to 2 freezes per year. If you, the membership, wishes to freeze the agreement you may do so by sending as long as a prescription hasn’t been filled within the 30 days you can roll your membership program over to a freeze status. If a prescription has been filled within the 15 days of the written request, your freeze will start the day after your billing cycle.

SUPPLEMENTS

Eternity Health Partners has recently partnered with a trusted pharmaceutical grade online supplement dispensary, Fullscript. Fullscript is the safest and most convenient way to purchase professional-grade supplement brands and products.

Supplement products that have been recommended by our Physician will arrive at your door quickly and in excellent condition. They take every precaution to ensure that temperature-senstitive products are stored and shipped according to the manufacturers specifications. We are thrilled to offer their service to you, as they are 100% in alignment with the level of client care and support as we are here at Eternity Health Partners.

A limited recommended over the counter supplements are available for purchased at our Eternity Health Partners. You are not obligated to purchase supplements from Eternity Health Partners.

MEET ELIZABETH BECKNER AND DAISHA ESCAMILLA

OUR CALIFORNIA LICENSED PHLEBOTOMISTS

Liz started doing personal blood draws after her son, Dashiell, was born with Zellwegers disease. His diagnosis required numerous blood draws and drug levels throughout his life. After sitting through long wait times at the labs followed by watching many different phlebotomists, many new to the job, struggle to find his veins, often poking him multiple times, as a seasoned phlebotomist, Liz decided to draw his blood at home and hand deliver it to the lab myself. Soon after she began doing personal touch blood draws so she can now offer that same personalized service to you.

Liz graduated from Pepperdine University with a degree in Biology in 1994. She moved to Santa Barbara and earned her phlebotomy certificate in 1995. Liz began working at St. Francis Medical Center in the Physical Therapy Department and quickly transferred to the Laboratory. Perfecting her venipuncture techniques on all floors of the hospital, including post-operative patients, pregnant women going into labor, sick or premature babies, and emergency room patrons, she then improved her patient care skills working for internal medicine doctors, one of whom was the Medical Director of Hospice and won Doctor of the Year. In 1999, Liz started working with a group of local retinal surgeons. Her primary job consisted of injecting dye into arm veins and photographing its flow through the patients retinal vessels. When patients are worried about their diagnosis or apprehensive about the procedure because of difficult veins, her quick, almost pain free needle stick and kind but professional bed side manner would put them at ease.

The various settings and patients Liz has encountered over the years have enabled her to become very skilled at finding the smallest and most fragile of vessels. Her specialities include infants and the elderly, diabetics of all ages, patients with rolling veins, on blood thinners or chemotherapy. She lives in downtown Santa Barbara, travels from Goleta to Carpintera and brings all the necessary supplies directly to you. All she needs from you is a lab request and your home or office becomes an instant draw station. Appointment times are very flexible, and she is friendly, reliable and punctual. Be kind to your arm and treat yourself to the luxury blood draw you deserve!!!

MEET MICHAEL BROOKINS; BS, CNMT, ABAAHP

Mike brings more than just a medical perspective to his clients; at the age of 14, he began an athletic career as a pole-vaulter that would end up taking him on an incredible journey over the next 13 years of his life. Throughout his athletic journey Mike was constantly working to find the ideal balance of fitness and proper nutrition in order to maximize his performances. Combined with his medical knowledge the years of athletic experience provide a unique and successful combination that serve to benefit his staff and clients.

“After I shifted the focus of my practice from Sports Medicine into Nuclear Medicine specifically Interventional Endocrinology. I found myself questioning our western medical paradigm…Here I was performing thyroid cancer and Hyperthyroidism treatments with harmful radical thyroid radioactive ablations which really made me take a hard look in the mirror and become more of a researcher to find safer and more effective options to better treat my clients. During my research I realized that there were very few, if any, medically directed programs focused towards achieving a “Healthier Lifestyle” rather than typical “Diet” fads.” A few years later after training and working with some of the worlds foremost Bio-identical Hormone Replacement specialists I became Board Certified in Anti-Aging and Regenerative Medicine. Sort of just found my passion and the answers I had been looking for with this new specialty finally after years of treating people with the Band-Aid “fix it when its broken” mentality instead of at the root cause.”

Mike has spent the last 10 years in Anti-Aging, Functional and Regenerative Medicine. He has trained over 120 physicians in Anti-Aging and Regenerative Medicine and consulting for their practices. Today his vision is a reality, and clients from all of the world come to benefit from the successful programs that have been established at Age Management Institute Santa Barbara. He firmly believes in educating the patient about the importance of good nutrition, hormone balance, stress management, and regular exercise is the key to aging well and having a high quality of life well into your golden years.
